NSW Health

UX & UI | Patient Journey Modelling Tool

Project Summary

The project aims to visualise the journeys of patients within the NSW health system for NSW Ministry of Health employees and their partners, who may or may not have a data analytical background. The access to and visualisation of patient journey data has huge potential to help improve patient outcomes through planning, monitoring and evaluating existing health programs and initiatives.

Project Outcome:

Through stakeholder workshops, end user consultations, user evaluation and iterations of the design we were able to deliver an interactive software tool for interrogating and viewing data on patient journeys. The tool allows users to:

  • Easily create and customise cohorts of interest

  • Compare multiple cohorts side by side

  • Select pre-defined graphs to view cohort data

  • Make small customisations to graphs

  • Save and export graphs and underlying data.

Project Highlights

  • The inclusion of pre-set graphs gives users inspiration to create new research questions and look at new trends and patterns with the tool.

  • Filters offer a new level of transparency to users about the available data.

  • Users were enabled to customise the appearance of graphs so they are presentation ready.
